Semaglutide and other GLP-1 medications have become widely used in clinical practice as effective tools for obesity and glycemic control. At the same time, millions of people live with hypothyroidism and rely on levothyroxine to keep their hormones balanced.

How GLP-1 interferes with thyroid hormone absorption

GLP-1 agonists act on intestinal receptors that are also involved in medication absorption. A study by Du et al. published in Cardiovascular Diabetology (2025) examined older adults on levothyroxine who initiated GLP-1 therapy and found that the interaction can alter thyroid hormone levels through changes in absorption and gastric emptying. GLP-1 slows gastric emptying, meaning the stomach takes longer to release the thyroid medication into the intestine where it would be absorbed.

In practice, this delay means that the levothyroxine you take on an empty stomach may not be fully absorbed if the GLP-1 was injected in the preceding or nearby hours. The result shows up in your test: TSH rises even though your thyroid medication dose has not changed.

Spacing between doses: a simple rule with a direct effect

The most recommended solution by manufacturers and most specialists is straightforward: separate the levothyroxine dose from the GLP-1 injection by at least four hours. You wake up, take levothyroxine with a glass of water, and wait at least thirty minutes before eating or drinking anything. The GLP-1 injection is reserved for the evening, four hours after your last meal and well before bedtime.

If your GLP-1 injection time is in the morning, the logic reverses: levothyroxine goes to the evening, separated by at least four hours from the injection.

The FDA warning about medullary thyroid carcinoma

In 2021, the FDA updated the semaglutide label to include a warning about the risk of medullary thyroid carcinoma. The decision was based on rodent studies that showed tumor development in the thyroid gland at high doses. So far, there is no confirmation that the same occurs in humans, but the warning remains as a precautionary measure.

The contraindication is clear for people who have a personal or family history of medullary thyroid carcinoma or who have Multiple Endocrine Neoplasia type 2.

Thyroid tests during GLP-1 treatment

TSH needs to be monitored more frequently when you start or change the dose of a GLP-1. The recommendation is to repeat the test between four and six weeks after any change.

The main tests your endocrinologist will usually order are TSH, free T4, and in some cases calcitonin. Keeping all of this recorded in one place makes appointments easier.

What to ask your doctor before combining medications

First, ask what the best time is to take levothyroxine in relation to your GLP-1 injection in your specific case. Second, ask how often thyroid tests need to be repeated. Third, ask whether there are any new symptoms you should watch for.
