Thyroid Medication and GLP-1: Interactions You Need to Know

Jun 20, 2026·2 min read·0 views·Equipe Editorial PeptPro

Patients on levothyroxine or methimazole need specific monitoring with GLP-1. Learn about drug interactions and how to organize your medication routine.

If you are on thyroid medication and starting GLP-1 treatment, there are specific considerations that apply to your situation. These are practical issues that come up frequently in clinical practice.

Levothyroxine and GLP-1

Levothyroxine is absorbed in the small intestine, and GLP-1 slows gastrointestinal motility. This means that if you take levothyroxine at the same time as your GLP-1 injection or close to a large meal, you may absorb less of the thyroid hormone than expected.

Studies have shown that GLP-1 can increase the absorption of levothyroxine in some cases, which is the opposite of what you might expect. The interaction is complex and individual. Some patients need higher doses of levothyroxine when on GLP-1; others need lower doses.

The practical solution is timing separation. Take levothyroxine first thing in the morning on an empty stomach, at least 30 to 60 minutes before anything else, including your GLP-1 injection or breakfast.

Methimazole and other antithyroid medications

Methimazole is less affected by absorption timing because it works by blocking thyroid hormone production rather than relying on intestinal absorption. However, if you are on methimazole, you still need regular TSH monitoring when starting or changing GLP-1 treatment.

TSH monitoring schedule

Get a TSH test before starting GLP-1. Then repeat at 6 weeks and 12 weeks after starting. If values are stable, you can move to every 3 to 6 months.

PeptPro helps you log test results with dates and track trends. See the app here.

Bring your PeptPro log to your endocrinologist and you have a clear record of TSH over time, which makes dose decisions more informed.

Checklist for your doctor visit

Before you see your endocrinologist, write down your current levothyroxine dose, the time you take it, the time of your GLP-1 injection, and your most recent TSH results. Note any symptoms you have noticed since starting GLP-1.

This is more useful than showing up and saying you feel a certain way. Your doctor needs structured data.

PeptPro organizes all of this for you. Start tracking here.
